If you are suffering from pain, talk to your doctor about a referral or call us for more information at 408.364.6799. This referral can come from the patient, a primary care physician, another medical specialist or a workers' compensation attorney or carrier. We evaluate each referral to determine the most appropriate type of consultation with our clinical staff. We ask each referral to provide our office with the most complete medical history available, including progress notes, imaging and diagnostic reports. We also need patients' demographic information so we may contact the patient. If the patient is covered by workers' compensation or SCCIPA HMO, we request authorization be obtained and faxed along with the referral. When our office receives the necessary medical information, we will contact the patient for a consultation. Patients will receive a packet of information in the mail, or they can download information from our web site. In order for our physicians to fully assess each patient's history, it is important for the paperwork to be completed before the initial consultation. At the consultation, the physician and the patient will discuss a treatment plan. We do not prescribe medication at the initial consultation. Follow-up appointments, injections or other procedures will be scheduled after the initial consultation.
